What forms do I need to complete?
If you complete the online form it will capture all information needed.
If you download the forms, you will need to complete:
- Notice of Accident - if you were the owner or driver of a vehicle involved in the motor vehicle accident.
- Application for Benefits - if you were injured in a motor vehicle accident.
If you are unsure, please phone us on 1800 006 224 and we will help guide you through the process.
When someone is fatally injured
Please call the MAIB on 1800 006 224 for support with lodging a claim. A claims officer will be guide you throughout the process.
What happens next?
Once you have lodged your claim, we will contact you within 5 working days of receipt of your claim to notify you of the progress of your claim.
Lodge an MAIB claim online
If you have been injured in, and/or received medical treatment due to a motor accident, you can lodge a claim with us online.
If you are unable to lodge your claim due to incapacity, it may be lodged by an assisting person. That person’s details will need to be provided as part of your claim.
Before you start your claim
Before you start, you will need the following information to lodge a claim:
- Your details – name, contact and bank information
- Accident details – date, time, location, circumstances and vehicles involved
- Details of injuries sustained in the accident
- Details of any previous personal injury claims or pre-existing medical conditions
- Details of police attendance or date reported to police
- Employment details if you have had time off work due to the accident
- Any documents you want to attach to your claim e.g. Medical invoices, medical certificates.
If you don’t have all the information listed, just complete the form the best you can.
